The FSMG Fertility Diagnostic evaluation costs just $700 and includes:

• New patient/partner consultation
• Return office visit(s) (up to 3)
• Ultrasound(s) (up to 2)
• Complete semen analysis (up to 2)
• AMH (Anti-Mullerian Hormone) blood test to assess ovarian reserve
• Sonohystogram (to evaluate the uterus)

We are committed to your completion of a diagnostic evaluation. We have bundled these services for a savings of over $850 if you choose to purchase the case rate on the day of the new patient consultation. The fee for this case is $700 and includes the fee for your partner. At your return visit, your physician will discuss:

• The results of the diagnostic evaluation
• Any underlying health care issue related to conception
• Treatment options such as:
  • Ovulation Induction
  • Artificial Insemination (AI)—also known as intrauterine insemination (IUI)
  • In Vitro Fertilization (IVF)
  • Pre-Implantation Genetic Screening (PGS)
  • Third Party Reproduction (egg donation, sperm donation, and surrogacy)
